After Clicking on Request
It will ask you following:
End User Name = Who currently ownsthe number from the Tier 1? carrier
Carrier Name = The Name of the Carrier who currently ownsthe number
Street Number = Street number which is stated on the bill sent by Carrier
Street Name = Street name which is stated on the bill sent by Carrier
Street Suffix = Street Suffix as on Bill
Post Direction= Post direction details if any
Suite / Floor = Your suite or floor number
City, State = As explained on the bill
Zip Code = As written on the bill
Country = As written on the bill
Cell Phone = Your Cell Phone number
Caller Id Info= You can specify the Caller Id info, if you want to give any
Authorized Nam= If the number which is being ported is on the company’s name then provide us the name of the person who use to deal with thecarrier
Last Month Bill = Upload the last month bill copy which you have received from carrier
Any Other Supported Document = You can upload the LNP form of DIDx (Lnp form is on http://www.didx.net/lnp) or any other Supported Document
Once this is done click on Continue and it should tell you that your request is successfully Submitted.
